LPN/LVN - Long Term Care/SNF
LPNs are often the "frontline" caregivers, focusing on direct patient care for stable patients with predictable outcomes. Monitoring patient health: regularly checking and recording vital signs (blood pressure, temperature, pulse, respiration) and observing patients for any changes in physical or mental condition. Basic patient care: assisting patients with activities of daily living (ADLs), including bathing, dressing, eating, and personal hygiene. Medication administration: administering most types of medications (oral, topical, intramuscular, subcutaneous) and immunizations, following the "five rights" of administration (right dose, route, time, site, patient response).
Wound care: performing basic and some specialized wound care procedures, such as cleaning, dressing changes, and applying compression therapy, often following an established care plan. Clinical procedures: performing tasks like inserting urinary catheters, collecting specimens (urine, blood, sputum)
