• Provide direct nursing care to residents per the plan of care, under supervision
• Observe and document patient status, including vital signs and response to treatments
• Administer prescribed medications and report any adverse reactions or concerns
• Assist residents with activities of daily living and mobility, ensuring safety and comfort
• Maintain accurate charting and documentation in the electronic medical record per facility policy
• Collaborate with the interdisciplinary care team to communicate changes in condition and update care plans as needed

• Current Licensed Practical Nurse (LPN) license in good standing
• Ability to work the listed schedule:
  Fridays, Saturdays, and Sundays, 07:00–19:30
• Experience in long‑term care or skilled nursing settings preferred
• Strong communication, observation, and documentation skills
• Ability to pass background check and drug screening as required
• Eligibility to work in the United States and meet state‑specific licensure requirements
