LVN ​/ LPN – LTC​/SNF

Nursing Position in Long-Term Care Facility

Provide compassionate nursing care to residents in a long-term care or skilled nursing facility setting. Ensure patient safety, comfort, and well-being through quality clinical practices and adherence to facility protocols.

Responsibilities:

• Administer medications and treatments accurately following physician orders.
• Monitor patients’ health statuses and report any changes promptly.
• Document patient observations and care activities in accordance with regulatory standards.
• Collaborate with interdisciplinary team members to develop and implement care plans.
• Maintain a safe and clean environment, complying with infection control policies.
• Educate residents and their families on health management and care plans.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1-year recent experience in LTC/SNF settings.
• Ability to effectively document and communicate patient information.
• Demonstrate strong clinical skills and attention to detail.
• Must be vaccinated for COVID-19 or have valid exemption documentation.
• Ability to work flexible hours and adapt to a fast-paced environment.
