LVN ​/ LPN – LTC​/SNF

Long-Term Care Nursing Position

Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting. Support patient well-being through monitoring, treatments, and collaboration with the healthcare team.

Responsibilities:

• Administer nursing care and treatments to residents in accordance with physician orders.
• Monitor vital signs and assess patient conditions regularly.
• Document patient observations and care activities accurately.
• Collaborate with registered nurses and medical staff to develop care plans.
• Ensure compliance with safety protocols and infection control procedures.
• Maintain accurate records of medication administration and patient progress.

Qualifications:

• Valid LVN or LPN license in the state.
• Prior experience in LTC or SNF environments preferred.
• Ability to adhere to facility policies and procedures.
• Strong communication skills and compassion for patient care.
• Must have current COVID-19 vaccination or approved exemption.
• Ability to bring a valid state or federal issued  wear a photo badge at all times.
