LVN ​/ LPN – LTC​/SNF

Nursing Position in Long-Term Care Facility

Provide quality nursing care to patients in a long-term care or skilled nursing facility setting. Ensure the delivery of compassionate, patient-centered services while adhering to clinical protocols and standards.

Responsibilities:

• Assess patient health conditions and document findings accurately.
• Administer medications and treatments as prescribed by physicians.
• Monitor vital signs and respond to patient needs promptly.
• Collaborate with interdisciplinary team to develop and update care plans.
• Ensure compliance with safety standards and infection control protocols.
• Maintain detailed, accurate documentation of patient care activities.

Qualifications:

• Hold a valid LVN or LPN license in the state of employment.
• Possess current CPR and immunization records, including COVID-19 vaccination or approved exemption.
• Demonstrate excellent communication and interpersonal skills.
• Have prior experience working in LTC/SNF environments.
• Ability to follow detailed instructions and protocols effectively.
