Santa Fe Recovery Center, Inc. is seeking a Case Manager in New Mexico to coordinate continuum of care for clients at high risk for HIV. You will work within an interdisciplinary team to maximize resource utilization and ensure successful transition back to the community.
Responsibilities include education on HIV prevention, care coordination, and referrals, with ongoing follow‑ups and strict confidentiality. Valid NM driver’s license and experience with at‑risk populations are required.
