Role Overview
We're seeking Primary Care NPs to provide on‑site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.
What You'll Do- Routine & urgent assessments; chronic disease management
- Post‑discharge follow‑ups, medication reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same‑day documentation; CMS/regulatory compliance
Full‑time position with a structured weekday schedule. Providers deliver on‑site care across Stockton, Modesto, and Amador with strategically clustered routes, so you spend more time with patients and less time on the road.
Requirements- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility‑based experience preferred; EMR proficiency
- Annual compensation range: $140,000-$175,000 (based on experience, schedule, and patient volume)
- Additional earning opportunities available based on coverage and panel needs
- W-2 with full benefits available; mileage reimbursed per policy
- Paid Time Off
