Position Summary
UTMB is recruiting a full‑time advanced practice provider within the Division of Geriatrics to provide patient‑centered care to medically complex and functionally limited older adults in the ambulatory setting. This position emphasizes high‑quality, age‑friendly clinical care, active participation in teaching, and engagement in the academic mission of the department. The successful candidate will join a dedicated team of faculty focused on improving health outcomes for older adults and advancing excellence in geriatric education.
EssentialResponsibilities
- Provide comprehensive outpatient care for older adults, addressing complex medical, cognitive, and functional conditions.
- Provide primary care for patients in the geriatric house call program, as well as residents living in both independent and assisted living settings.
- Perform individualized history and physical examinations for diverse populations inclusive of social determinants of health.
- Order and interpret diagnostic testing in accordance with the relevant health concerns of the patient.
- Precept and mentor nursing and medical students.
- Develop a comprehensive plan of care that encompasses evidence‑based practice guidelines, practice protocols, Medicare, Medicaid, and other state and federal regulations.
- Prescribe appropriate pharmacological and non‑pharmacological treatments.
- Provide appropriate education for current health diagnoses as well as anticipatory guidance for health promotion and disease prevention applicable to the individual’s health and socioeconomic status.
- Monitor plan of care for cost‑effectiveness and the achievement of expected improvements, while openly seeking feedback from patients, significant others, colleagues, allied health, appropriate referrals, and collaborating physicians to enhance quality outcomes.
- Work independently to full scope of practice in compliance with the Texas Nurse Practice Act with acknowledgement of collaborating physician availability for consultation and collaboration.
- Collaborate with interdisciplinary teams—including nursing, pharmacy, social work, and rehabilitation—for optimal care of older adults.
- Contribute to curriculum development, clinical skills training, and geriatric‑focused academic programs.
- Master’s or Doctoral degree in Nursing or Physician Assistant Studies from an accredited program.
- Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA‑C) with current license in the State of Texas.
- Current Adult‑Gerontology Acute Care Nurse Practitioner (AGACNP) certification or Physician Assistant (PA‑C) with Certificates of Added Qualifications in Acute Care.
- Experience in providing acute care to geriatric or medically complex populations.
- Experience in an academic medical center or teaching environment.
- Demonstrated involvement in scholarly activity, quality improvement projects, or curriculum development in geriatric care.
- Strong interpersonal and communication skills with a collaborative, team‑oriented approach.
