In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Manchester, NH
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in the Manchester, NH area. The role includes conducting visits, reviewing medication and medical history, performing physical evaluations, and ordering diagnostic tests as needed.
Base Pay Range$87,035.00 / yr – $ / yr
Job HighlightsThe visit lasts approximately 45 minutes and includes time to connect one‑on‑one, answer health questions, review medication and medical history, perform a physical evaluation, and, when ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad and an intuitive clinical workflow. You are not required to prescribe medication or alter people’s current treatment regimen.

- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits when required.
- Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry, DEE, bone density screening, etc.
As an in‑home clinician, your primary workplace is the private residence of health‑plan members. Candidates must be comfortable and safe performing duties in homes that may contain pets, secondhand smoke, or potential allergens.

- Board certification (required)
