Position Summary

The Resident Services Coordinator will report to the Resident Services Director and will assist by helping manage the Wellness Department and prioritizing resident health. Responsibilities include overseeing medication administration and record keeping, scheduling staff, developing daily tasks, initiating and maintaining emergency packets, and ensuring the organization of resident charts.

Essential Duties and Responsibilities
  • Monitor team members’ medication administration, including frequent audits of medication records, ordering medications from pharmacy, and ensuring proper storage and care.
  • Record all significant changes regarding resident health and communicate to appropriate parties, including reactions to drugs, treatments, and incidents.
  • Assist with medication management as needed, including med pass and documentation.
  • Update medical charts and care plans, maintaining daily resident records in accordance with company policy and procedures.
  • Perform work assignments and attend required in‑service training; report concerns and problems to supervisor.
  • Incorporate State Department of Social/Community Services/Public Welfare/Licensure requirements during in‑service training and orientation.
  • Review and implement incident reports; evaluate for intervention to minimize risk of further occurrences.
  • Schedule Personal Care Assistants, develop daily tasks, and make assignments.
  • Ensure medical supplies are always in stock; take inventory and order supplies.
  • Follow up weekly on bath schedules and vital signs (blood pressure, weight, respiration, etc.).
  • Act as point of contact during shift for communication with supervisors, doctors, and families.
  • Contribute to community relationships by demonstrating cooperation and professional conduct with residents, families, fellow team members, physicians, pharmacies, and vendors.
  • Contribute to team effort by being flexible in work assignments, furnishing support, taking initiative, and understanding how this position complements other community positions.
  • Perform activities on schedule while maintaining regular attendance and punctuality within specified tolerance.
  • Maintain an appropriate work pace for the given workload.
  • Maintain all health equipment for safety and proper function.
  • Lift and/or transfer residents as needed.
  • Correct environmental hazards in an appropriate and safe manner.
  • Check resident files weekly for appropriate and necessary signatures.
  • All other duties as assigned.
Supervisory Responsibilities
  • The incumbent in this position will have no direct reports.
Education, Experience, and Credentials
  • At least 18 years of age.
  • Required health screening, TB clearance, and criminal record clearance.
  • First Aid/CPR certification (any special licenses or certificates as required).
  • High school diploma or equivalent and prior care experience in an assisted living community or skilled nursing facility.
  • Preferred certified nursing assistant certification.
  • Ability to speak, read, write, and understand the English language fluently.
  • Ability to work additional hours, which could include evenings and weekends.
Knowledge, Skills, and Abilities
  • Understanding of the unique needs, challenges, and preferences of elderly residents in an assisted living facility.
  • Familiarity with relevant healthcare regulations and compliance requirements.
  • Knowledge of designing, implementing, and evaluating wellness programs tailored to elderly residents.
  • Awareness of health and safety protocols to ensure resident and staff well‑being.
  • Understanding of emergency protocols and procedures for crisis situations.
  • Knowledge of medication administration, storage, and adherence practices.
  • Effective communication with residents, families, staff, and healthcare professionals.
  • Ability to identify challenges and find practical solutions to enhance resident well‑being and program effectiveness.
  • Ability to manage multiple tasks, schedule, and maintain records efficiently.
  • Proficiency in using software and technology for program management, documentation, and communication.
  • Flexibility and adaptability in a dynamic healthcare environment.
  • Ability to conduct wellness assessments and individualized care plans for residents.
  • Working effectively with interdisciplinary teams, including nurses, therapists, and administrators.
  • Ability to handle conflicts and difficult situations with diplomacy and professionalism.
  • Willingness to stay updated on industry trends and best practices in elderly care and wellness programs.
Physical Requirements
  • Ability to move residents and support their physical needs.
