Position SummaryThe Health Navigator is responsible for supporting refugees and immigrants in accessing comprehensive healthcare services and building client self‑sufficiency in navigating healthcare systems. This limited‑term position is funded through a grant agreement until September 30, 2026, and is contingent upon funding extension.
Responsibilities- Provide case management services, individual coaching, and connect clients to resources and benefits.
- Create stability using a strengths‑based approach to address client needs and promote long‑term self‑sufficiency.
- Ensure services are delivered within grant‑specified time frames and with appropriate documentation.
- Manage assigned cases, ensuring client experience, program outcomes, timely reporting, and collaboration with colleagues.
- Assess participant needs, strengths, and goals and establish comprehensive action plans.
- Deliver client‑centered health navigation services to newly arrived Afghans, including enrollment in insurance and health benefits (Medicaid, Social Security).
- Ensure clients establish primary care and have access to needed specialty services.
- Assist with booking appointments, scheduling transportation, and arranging interpretation through insurance.
- Identify appropriate service providers, coordinate referrals, and advocate for ongoing access to services and care.
- Provide individual coaching to promote self‑sufficiency in independently navigating the health‑care system.
- Provide transportation to required appointments when necessary.
- Work with cross‑departmental staff to coordinate client integration and referrals across programming.
- Support and build partnerships with local providers and community organizations addressing healthcare needs.
- Maintain timely and accurate case notes, track program data, and stay current on program requirements.
- Document participant services and activities thoroughly and in accordance with program and World Relief requirements.
- Maintain accurate, timely, and compliant casefiles.
- Update client information in protected online databases (Dynamics, Teams, etc.).
- Conduct regular recurring meetings to monitor progress and connect clients to resources.
- Achieve grant requirements and deliverables through high‑quality services.
- Participate in ongoing training to improve service delivery and ensure compliance.
- Present case status and progress to the Program Manager.
- Collaborate with World Relief staff and external providers to ensure holistic case management.
- Attend meetings as instructed by supervisor.
- Perform other duties as assigned.
- Mature and personal Christian faith.
- Committed to World Relief’s mission, vision, and values.
- Desire to serve and empower the Church to impact vulnerable communities.
- Able to affirm and acknowledge World Relief’s Core Beliefs, Statement of Faith, Christian Identity, and the National Association of Evangelicals’ For the Health of the Nation document.
- Bachelor’s degree in social work or related field of study; or equivalent work experience.
- Cross‑cultural experience.
- Strong communication skills, fluent in written and spoken English.
- Local travel and transportation of program participants required; personal car usage.
- Fluency in another language preferred.
