Licensed Practical Nurse

Magnolia Ridge Center, a skilled nursing and rehab facility in Barre, VT, seeks a Licensed Practical Nurse to deliver compassionate, resident-centered care. The LPN will administer medications and treatments, monitor resident conditions, and support individualized care plans in collaboration with RNs, therapists, and physicians. Responsibilities include documentation in EHR systems, assisting with rehab and long-term care goals, and upholding infection control and safety standards.

Responsibilities

• Provide direct nursing care to residents under RN and provider direction
• Administer medications and treatments per physician orders and facility policy
• Monitor, document, and report changes in resident condition using EHR systems
• Participate in developing and implementing individualized care plans
• Support rehab and long-term care goals through collaboration with the interdisciplinary team
• Educate residents and families on care, treatments, and health management
• Maintain infection control and safety standards at all times
• Assist with admissions, discharges, and transfers, including assessments and documentation
