Provide skilled physical therapy services to SNF patients, including assessment, treatment planning, and intervention to support recovery and management of chronic conditions. Collaborate with healthcare team to ensure optimal patient care.

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Experience preferred in skilled nursing and geriatric populations
