Occupational Therapist

We are looking for an experienced Occupational Therapist on behalf of Hereford Regional Medical Center

Hereford Regional Medical Center is a community-centered healthcare organization serving Hereford and the surrounding region. Our rehabilitation team provides high-quality therapy services across outpatient, inpatient, and swing bed settings. As a rural healthcare facility, we take pride in building strong relationships with our patients and delivering personalized care to a close-knit community.

We serve a diverse patient population, including a large number of Spanish-speaking patients, making cultural awareness and effective communication essential components of our care model.

The Occupational Therapist is  responsible for evaluating, planning, and administeringoccupationaltherapy treatments designed to help patients achieve maximum independence, improve functional performance, and enhance quality of life. This position works with patients across multiple settings, including outpatient rehabilitation, acute inpatient care, and swing bed services.

• Full-time position
• 45-minute treatment sessions
• Daily patient schedule from8:15 AM to 4:45 PM
• 1-hour lunch break
• Occasional weekend rotation coverage for inpatient and swing bed patients

Location: Remote

• Perform comprehensiveoccupationaltherapy evaluations and reassessments.
• Develop individualized treatment plans based on patient needs, goals, and physician orders.
• Deliver evidence-basedoccupationaltherapy interventions to improve functional independence and quality of life.
• Treat patients with a variety of orthopedic, neurological, post-surgical, developmental, and medically complex conditions.
• Educate patients and caregivers regarding functional strategies, home programs, adaptive equipment, and safety recommendations.
• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely.
• Collaborate with physicians, nursing staff, therapy colleagues, case management, and other healthcare professionals.
• Participate in discharge planning and care coordination activities.
• Monitor and assist with therapy authorization timelines to support uninterrupted patient care and treatment scheduling.
• Maintain compliance with federal, state, and organizational standards of practice.
• Contribute positively to departmental quality improvement initiatives and team development.

• Required
  • Graduate of an accredited
    
    Occupational
    
    Therapy program.
  • Current Texas Occupational Therapist license  or eligibility for licensure.
  • Current BLS certification or willingness to obtain upon hire.
  • Strong clinical reasoning, communication, and documentation skills.
  • Ability to work independently and collaboratively within a multidisciplinary healthcare team.
• Preferred
  • Experience in outpatient rehabilitation, acute care, swing bed, or rural healthcare settings.
  • Experience working with geriatric, orthopedic, neurological, and post-operative patient populations.
  • Spanish language proficiency or the ability to effectively work with interpreters when caring for Spanish-speaking patients.
  • Familiarity with insurance authorization processes and managed care requirements.
