This role is for a Registered Nurse (RN) providing home-based care to pediatric and/or adult patients in their own homes.
Key Responsibilities Include- Utilizing the nursing process to plan, implement, and evaluate patient care
- Initiating and modifying care plans based on patient needs
- Administering medications and treatments, monitoring effectiveness
- Responding to changes in patient condition and reporting to physicians or supervisors
- Providing healthcare education, care coordination, and discharge planning
- Current RN license for the practicing state
- Current health certification, PPD or chest X-ray, and BLS card
- Preferred: one year of RN experience
- Strong clinical judgment, attention to detail, effective communication, and ability to respond to patient needs.
