Position SummaryThe Rapid Rehousing Case Manager provides intensive case management services to chronically homeless individuals and families fleeing domestic violence. This role requires knowledge and experience in Housing First principles, mental health, addictive disorders, and benefits eligibility, particularly as they relate to homelessness.
The Case Manager will assist participants in transitioning from homelessness to stable housing through rapid rehousing programs. Responsibilities include facilitating successful housing placements, connecting participants to mental health and substance abuse treatment resources, and providing ongoing housing stabilization support. The Case Manager will also collaborate with community agencies, charities, and government programs to ensure participants receive comprehensive support services.
Qualifications Education- Bachelor’s degree in Social Work or a related field required
- Minimum of one (1) year of case management experience working with the homeless population
- Current Texas Social Work license preferred but not required
- Proficiency in MS Word, Excel, and Outlook
- Ability to work flexible hours as needed
- Valid driver’s license, insurance, and reliable transportation
- Willingness to transport program participants
- Ability to work with chronically homeless individuals with physical, mental, and/or substance use disabilities, including those with criminal backgrounds
- SOAR and Motivational Interviewing training preferred
- Provide intensive case management services using a Housing First model
- Conduct office and home visits with participants
- Complete intake and assessment processes; develop individualized short- and long-term service plans
- Conduct benefits eligibility screenings and assist participants in obtaining income, education, training, and treatment services
- Assist participants in obtaining critical documents and Documentation of Priority Status (DOPS)
- Provide outreach and engagement to unsheltered homeless individuals as needed
- Deliver crisis management and intervention services
- Maintain consistent follow-up with participants through biweekly meetings and monthly home visits
- Ensure contract compliance and verify participant eligibility for program services
- Utilize evidence-based practices, including motivational interviewing, trauma-informed care, and harm reduction
- Network with community agencies to coordinate service referrals and resource access
Collaborate with other case managers, outreach workers, interns, and volunteers to identify and meet participant needs - Act as a liaison among program participants, landlords, and partner agencies to enhance communication and service delivery
- Assist in resolving issues and developing solutions for participant success
- Facilitate Life Skills classes as needed
- Maintain accurate documentation and records of encounters, assessments, referrals, and statistical data in HMIS and U1R Housing databases
This position involves both office-based and fieldwork activities, including home visits and community outreach. Flexibility and adaptability are essential to meet the needs of participants and program requirements.
