• Flexible shifts available Monday through Sunday
• Set your own schedule according to your preferences
• Minimum weekly or bi-weekly hourly commitment required

• Perform thorough home health risk assessments, including annual evaluations and post-discharge visits for Medicare Advantage members
• Gather and assess medical histories, including prior and current conditions and medications
• Offer patient education on health conditions and medication management
• Recognize gaps in care and provide actionable recommendations to improve patient outcomes
• No requirements to develop treatment plans or prescribe medications

• Active Nurse Practitioner License in North Carolina
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of 6 months of nurse practitioner experience preferred
• MSN degree from an accredited nursing program

Compensation for this role ranges from $80 to $90 per completed evaluation, including a mileage stipend and compensation for administrative tasks.
