Medical Social Worker

Oak Street Health is seeking a compassionate Medical Social Worker to join our Healthcare & Medical Services team. In this role, you'll support adults on Medicare by coordinating psychosocial assessments, care plans, and community resource connections to address social determinants of health. You will collaborate closely with physicians, nurses, and care teams to reduce barriers to care, support treatment adherence, and advocate for patients and families.
