Guide Star Eldercare seeks a Nurse Practitioner to address dementia and neurocognitive disorders in nursing homes and assisted living facilities. You will travel daily from home, perform initial evaluations, ongoing therapy, and cognitive testing, and document services for each patient.
You will build strong relationships with facilities, patients, and families while supported by corporate clinical leadership and in-house scheduling, coding, and billing teams.
