Responsibilities:Provide effective, equitable primary care to adult Medicare patients, including routine and acute visits, wellness exams, care coordination, and patient education. Practice independently while collaborating with physicians and a multidisciplinary team. Lead population health initiatives, assist with phone triage and outreach, conduct home visits if needed, and participate in promotional activities. Work closely with operational leadership and support a network of specialists and hospitals.
Qualifications:Master's or Doctorate in Nursing, with active certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner, and valid state license with DEA registration. Must have US work authorization. Preferred experience in primary care, understanding of HCC and ICD-10 coding, and bilingual skills are advantageous.
