Description POSITION SUMMARY

The Part-Time Social Worker – Human Trafficking provides direct social work support, care coordination, resource navigation, and referral follow-up for individuals who are survivors or suspected survivors of human trafficking. The position works collaboratively with the Clinic’s Human Trafficking Social Worker, healthcare providers, shelters, case-management partners, advocacy agencies, and community and faith-based organizations to support continuity of care. The role also assists with program documentation, data collection, training, outreach, and other activities that support the Anti-Human Trafficking Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Provides direct social work support to individuals who are survivors or suspected survivors of human trafficking, including screening support, care coordination, referrals, follow-up, and resource navigation.
  • Coordinates appropriate medical, dental, behavioral health, and social-service referrals and assists with continuity of care across Clinic and community providers.
  • Works closely with shelters, case-management partners, healthcare providers, advocacy agencies, and community and faith-based organizations to facilitate services for referred patients.
  • Serves as a liaison with established community partners and assists with communication and coordination related to patient referrals and program activities.
  • Provides timely follow-up with patients and partner organizations regarding referrals, services, and identified barriers to care.
  • Assists with collection, maintenance, and organization of Anti-Human Trafficking Program data to support quality assurance, operational tracking, and required reporting.
  • Maintains accurate and timely patient, referral, outreach, and program documentation in accordance with Clinic requirements.
  • Assists with scheduling, coordination, preparation, and delivery of human trafficking trainings for healthcare providers, advocacy agencies, community partners, and community members.
  • Participates in community outreach activities, educational events, tours, panels, vendor opportunities, and train-the-trainer activities as assigned.
  • Identifies and coordinates available community and donated resources to address barriers to care, including transportation, food, clothing, hygiene, communication, and other essential support needs.
  • Assists with preparation, organization, setup, breakdown, and maintenance of materials used for outreach, education, and community events.
  • Provides program information, documentation, and data needed to support grant reporting and funding compliance, as requested by program leadership.
  • Supports established community partnership activities and approved agreements and assists with implementation and coordination as directed by program or executive leadership.
  • Monitors assigned cases, referrals, and program tasks and promptly communicates operational or patient-care concerns to the appropriate supervisor or program lead.
  • Works independently within assigned responsibilities while maintaining effective communication and collaboration with the Human Trafficking Social Worker, clinical staff, and leadership.
  • Maintains confidentiality of patient information and complies with HIPAA, OSHA, Clinic policies, professional standards, and applicable program requirements.
  • Performs other related duties as assigned.
Requirements QUALIFICATIONS, EDUCATION, AND EXPERIENCE
  • Master’s degree in Social Work required.
  • Two to three years of healthcare, social work, case management, or related experience preferred; human trafficking experience strongly preferred.
  • Bilingual English/Spanish required.
  • Demonstrated ability to work effectively with trauma-affected, vulnerable, and underserved populations.
