Summary of Position: The Special Population Navigator assists patients in meeting their healthcare needs by enrolling them in appropriate healthcare payor sources (Medicaid, Medicare, ACA Insurance), connecting uninsured/underinsured patients to pharmaceutical charity programs, and FQHC services as well as providing connection to other community resources based on their expressed needs. Additional duties include creating and maintaining relationships with community agencies serving the organization's patient population through community outreach and engagement.

As the Gaston County SHIIP Coordinator, the Special Population Navigator is responsible for all SHIIP training, reporting, and outreach. Additionally, the Special Population Navigator provides support when needed for initiatives including but not limited to Justice Involved Transitions Program, Gaston County Laundry and Shower unit events, Hope Fest, and PIT Count.

Minimum Qualifications:
The following qualifications are the minimum necessary to adequately perform this job. However, any equivalent combination of experience, education, and training which provides the necessary knowledge, skills, and abilities would be acceptable, subject to any legal and/or regulatory requirements;
Must be able to sit, stand and walk for long periods of time;
Ability to read and understand the English language;
Ability to effectively maintain confidentiality of records and communicate with all levels of personnel.

Experience: Experience in a medical office setting required.

Bilingual (English/Spanish): Preferred but not required

Additional Required Skills: Knowledge of medical office software and comfort in using computers in daily work activity. Must have excellent verbal communication skills. Excellent organizational skills. Must be able to solve problems and have critical thinking skills. Understand and maintain patient confidentiality

Additional Preferred Skills:
Basic medical terminology. Proficient in use of all computer software utilized in the organization, including but not limited to Epic, Marketplace.gov, The Connector, SHIIP software (STAR), and TPC.

Education: High School Diploma or GED certificate required;
Bachelor's Degree preferred

Certification(s): SHIIP Counselor, ACA Certified Application Counselor, SOAR (?)

Core Duties

Enrollment/Navigation/Resource Connection

• Provide excellent customer service to all constituencies - community members, patients, fellow employees, community agency staff.

• Utilizing motivational interviewing techniques, assess individuals' current needs and resources.

• Based on information provided by individuals served, educate, enroll, and/or refer to appropriate resources.

SHIIP Duties
  • Lead SHIIP outreach, education, and enrollment activities for Gaston County
  • Recruit, onboard, train, and manage SHIIP volunteers
  • Document SHIIP activities utilizing the STARS system as required by NC Department of Insurance; be timely with reporting requirements
  • Meet or exceed program metrics as determined by NC Department of Insurance
Overall Administrative Requirements

• Remain compliant and current with all MAP, Medicare, ACA insurance, SOAR, FQHC, and community agency certifications, program guidelines, policies, procedures, documentation requirements, and reporting standards.

• Participate in QI initiatives to improve processes and minimize errors and variation.

• Maintain efficient workflow.

• Utilize data systems to update and communicate with staff in order to receive appropriate referrals and expedite patient processing

• Document activities in The Connector, TPC, Epic, STARS, and other means as requested by team leads and/or program manager.

• Develop and maintain positive relationships with community members and agencies through community outreach and engagement.
