San Stone Health & Rehabilitation is seeking a Certified Nursing Assistant (CNA) to join our dedicated team of health care professionals. The role supports residents’ daily living activities while ensuring safety and comfort under the direction of the Director of Nursing, RN, and LPN.

• Assist with activities of daily living (ADL) including bathing, grooming, and hygiene.
• Serve meals to residents and assist with feeding as needed.
• Provide or assist with resident personal care.
• Take and record vital signs.
• Support resident mobility and transfer as required.
• Follow facility policies, procedures, and infection control standards.
• Collaborate with RN, LPN, and interdisciplinary team to plan and implement resident care plans.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in ongoing education and training.

• Current Certified Nursing Assistant (CNA) certification.
• Valid state nursing license or license to practice (LPN) preferred.
