Home Care Agency Nurse

Responsible for developing and implementing home care services for program participants under direct supervision of the STLC Home Care Agency (HCA) Supervisor. All employees must be knowledgeable and compliant with STLC values of respect, integrity, accountability, compatible goals, and compassionate care.
Specifications- Education
:
Associate's Degree in Nursing preferred but not required. - Experience
:
At least 1 year as an RN, preferably in home or community health; supervisory experience desired. Minimum of 1 year working with a frail elderly population. - Licensed RN in NC, current, valid NC driver's license and vehicle.
- Must work effectively in a team environment and treat geriatric patients within scope of practice. Meet competencies approved by CMS before independent work.
- Medically cleared for communicable diseases; all immunizations up to date prior to direct participant contact.
- Eligible to care for patients of all age groups (neonate to geriatric).
- Use the nursing process to assess home care needs of frail elderly population and develop specific plans of care.
- Complete in-home initial assessments, re-assessments, and supervisory visits every 90 days.
- Perform in-home visits and nursing tasks as ordered by primary care provider.
- Maintain home-care guidelines by writing and updating policies and procedures.
- Maintain a safe and clean working environment by implementing procedures, rules, regulations, and requesting assistance when needed.
- Provide information to patients and health care team by answering questions and requests.
- Resolve patient needs by utilizing Home Care Coordinators and team strategies.
- Communicate any changes in participant condition to supervisor.
- Support and promote unity among the team and collaborate with co-workers and participants as a member of the interdisciplinary team.
- Assist in clinic setting as determined by HCA supervisor.
- Available on call evenings, after hours, and during times staff provide home care.
- Perform other nursing tasks as designated by supervisor, including duties under Home Care Nursing Supervisor.
- Maintain vaccinations for all participants and documentation in EMR.

- Other duties as assigned by HCA Supervisor.
