Nurse Practitioner – Palliative Medicine (Gaston & Cleveland Counties) – Full-Time
Carolina Caring is seeking a compassionate and skilled Nurse Practitioner to join our Palliative Medicine Team serving Gaston and Cleveland Counties. In this role, you will travel to patients’ homes, providing personalized, patient-centered care while supporting comfort, dignity, and quality of life.
Schedule: Full-time, Monday–Friday, 8:30 AM–5:00 PM.
Requirements EducationGraduate of an accredited school of nursing with a Master’s degree in nursing and appropriate advanced training required for licensure as a registered nurse practitioner. Current, unrestricted North Carolina Nurse Practitioner license. Must maintain CPR certification. Must have active DEA certificate.
ExperienceA minimum of two years in a Primary Care setting. Gerontology experience or certification preferred. Experience with CMS Chronic Care Management Services preferred.
