Position: Registered Nurse (RN)

We seek compassionate and skilled Registered Nurses (RNs) who deliver exceptional patient-centered care while enjoying the autonomy and flexibility of home health nursing.

Full-Time, Part-Time, or PRN opportunities available. Positions open facility-wide.

What You’ll Do
  • Provide skilled nursing care to home health patients in accordance with established care plans
  • Perform comprehensive patient assessments and ongoing evaluations
  • Develop, implement, and update individualized plans of care
  • Monitor patient conditions and communicate changes to physicians and care team members
  • Coordinate and oversee patient services to ensure continuity and quality of care
  • Educate patients and families regarding disease management, medications, treatments, and home safety
  • Supervise and direct licensed and non-licensed clinical staff as appropriate
  • Complete timely and accurate clinical documentation and charting
  • Conduct chart reviews for active and discharged patient records to ensure compliance and quality standards
  • Participate in after-hours and on-call coverage as assigned
  • Coordinate scheduling and staffing needs as required
  • Maintain patient confidentiality and comply with all HIPAA regulations
  • Stay current on nursing best practices, regulations, and clinical innovations
Requirements
  • Current Registered Nurse (RN) license in the State of Texas
  • Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or related healthcare setting
  • Strong clinical assessment and critical-thinking skills
  • Ability to work independently with minimal supervision
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management abilities
  • Commitment to maintaining confidentiality and professional integrity
  • Previous home health experience preferred but not required
