E. Lutz Medical Center in Saginaw, Michigan is recruiting for a Social Worker that serves in a full-time capacity within the Homeless Programs under the Social Work Services (SWS), which includes affiliated community-based outpatient clinics (CBOCs). This position is located at the Gaylord CBOC and incumbent will be expected to travel to/from community locations and Veterans homes in a 10 county radius from the CBOC, all within an hour drive of the duty station.

Major duties include, but are not limited to:

• Serving as the Healthcare of Homeless Veterans (HCHV) Social Worker, providing outreach, assessments, referrals, and case management services to homeless Veterans identified in the community.
• The HCHV Social Worker is typically the first Homeless Programs clinician to make contact with the literally homeless and at-risk Veterans.
• Serving as the entry point for Homeless services, and thus, provides VA a way to provide outreach, assist, and offer homeless Veterans an "open door" to the continuum of VA services.
• Providing outreach services as a key and essential element of the HCHV program, designed to reduce homelessness among Veterans by directly contacting those who are of the most vulnerable and underserved and engaging them in supportive and rehabilitative services.
• Working in the community to provide outreach services to homeless veterans at shelters, soup kitchens, and other agencies in the community serving the homeless.
• Performing triaging/screening as well as comprehensive biopsychosocial assessments, integrating input from the Veteran, their caregiver/family, VA records, as well as brief assessments when appropriate.
• Completing diagnostic tools including the Homeless Operations Management and Evaluation System (HOMES) assessment to ensure appropriate housing resources and additional treatment services are received.
• Educating/linking Veterans and/or their caregivers about/with appropriate VA and community resources to address their needs, both in individual and group formats.
• Providing crisis intervention services and referral to appropriate resources both within the VA and community.

Work Schedule:

Monday-Friday, 8:00am-4:30pm; additional tours of duty may be required based on needs of the agency

Pay:
Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Note this is only for new to federal service candidates; pay will be set following federal pay-setting procedures for current federal employees.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. This is only for new to federal service candidates.

Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
