Salary & Benefits
Salary range: $40.79 – $53.03 (Hourly Rate)
Placement within the salary range depends on factors such as relevant work experience and internal equity. For positions represented by a labor union, placement is guided by the rules outlined in the collective bargaining agreement.

The Physical Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
- A licensed physical therapist who complies with the Code of Ethics/Standards of Practice as outlined by their professional organization and by the State of Illinois.
- The position includes direct patient care, teaching, participation on appropriate committees and departmental process improvement.
- Serves as a role model for all therapists and clinical interns in all aspects of professional, clinical, communication, education and leadership activities.

- Presents a friendly, approachable, professional demeanor and appearance.
- Provides accurate information and timely updates to patients and customers and addresses questions promptly or identifies appropriate resources.

- Maintains primary responsibility over patient plan of care through timely collaboration with treating PT/PTA.
- Demonstrates teamwork by helping co‑workers within and across departments, communicating effectively, respecting diverse opinions, and acknowledging contributions.

- Manages work schedule efficiently, completing tasks and assignments on time.
- Contributes to continuous improvement opportunities and processes.

- Demonstrates essential skill competencies in selecting appropriate evaluation, treatment modalities, equipment and protocols; performs re‑evaluations per department guidelines.
- Assumes complete responsibility for patient management from initial contact to discharge, ensuring functional progress, safety, and treatment outcomes.
- Adheres to department policy regarding evaluations, progress notes, and discharge summaries; all notes documented in the electronic medical record (EMR).
- Completes daily notes reflecting patient response, performance, and assessment, meeting documentation requirements for reimbursement.
- Provides educational materials and methods appropriate to the patient’s level of understanding and motivation.
- Participates in committees, departmental processes, and special projects per clinical expertise and performance plan.
- Builds professional relationships with multidisciplinary teams (e.g., physician, nurse, case manager) and participates in internal and external marketing.
- Creates a culture of respectful communication through listening, seeking input, open conversation, approachability, and timely acknowledgment of contributions.
- Accepts constructive feedback and makes timely changes, evidenced by coaching forms and team feedback.
- Proactively completes non‑patient care responsibilities within established timelines (coverage notes, PTO/schedule change requests).
- Contributes to educational efforts (in‑services, journal clubs, new hire orientation).
