Schedule

Part-time evenings, 2x12s, every other weekend and holiday.

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must possess the ability to assess data reflective of the patient's status and to provide the care needed by adult and geriatric patient groups.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to creating a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnoses and interventions to achieve desired resident outcomes.
• Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals.
• Maintain current knowledge and competency in Nursing Practice and participate in the mentoring/orientation of staff.
• Provide leadership under the direction of the RN in the professional practice setting.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS certification through AHA or RQI.
• Minimum of 1 year experience as an LPN.
