LPN – Long Term Care

Schedule:

Full-time evening shifts. Every other weekend required. Candidates must be available to stay through the full holiday season and start on a Tuesday or Thursday.

Assignment Details:

• Contract Length: 27 weeks

Requirements:

• NY LPN Licensure
• BLS Certification (AHA or RQI required)
• 1 year of experience as an LPN in a long-term care facility

Roles & Responsibilities:

• Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
• Collect and communicate data relevant to residents' physical and psychosocial conditions to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant resident care plans involving residents, families, and healthcare teams.
• Maintain current knowledge and competency in nursing practice and participate in the orientation and mentoring of staff.
• Ensure customer satisfaction through collaboration with the resident, family, and interdisciplinary team.

Compliance / Notes:

Occupational exposure to blood borne pathogens is reasonably anticipated; required tasks may involve contact with blood, body fluids, or tissues.
