This role specializes in providing comprehensive care for adult and geriatric patients within a structured healthcare environment. The successful candidate will work closely under the guidance of a Registered Nurse and contribute to a collaborative care team.

Key Responsibilities of the LPN/LVN

• Collects data relevant to resident's physical and psychosocial condition and communicates findings to the RN and the Interdisciplinary Team.
• Contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care.
• Recommends revisions to the plan of care, collaborating with the resident/family and the Interdisciplinary Team.
• Maintains current knowledge and competency in Nursing Practice and mentors new staff.
• Considers safety, cost, and impact in the planning and delivery of care under the direction of the RN.

Requirements of the LPN/LVN

• Valid NY LPN Licensure
• BLS certification (AHA or RQI required)
• 1 year of experience as an LPN, preferably in a long-term care facility
