Key responsibilities of the Licensed Practical Nurse (LPN) include:

• Collects data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicates findings to RN and other team members
• Contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions
• Recommends revisions to the plan of care, ensuring all aspects of the resident's care have been addressed
• Maintains current knowledge and competency in Nursing Practice and participates in staff mentoring/orientation
• Considers factors related to safety, cost, and impact in planning and delivering care

Requirements of the Licensed Practical Nurse (LPN) include:

• NY LPN Licensure
• BLS certification (AHA or RQI)
• 1 year of experience as an LPN in a long-term care facility
