Seven Healthcare are seeking a Licensed Practical Nurse (LPN) for a travel position in Geneva, NY. The successful candidate will provide both direct and indirect nursing care, under the supervision of a Registered Nurse, in a collaborative environment, focusing on adult and geriatric patients.

Key Responsibilities of the LPN/LVN:

• Collect data on residents' physical and psychosocial condition, communicating findings to the RN and Interdisciplinary Team.
• Contribute to developing and revising culturally relevant care plans, ensuring resident outcomes are achieved.
• Recommend care plan modifications after evaluating resident outcomes, collaborating with the Interdisciplinary Team.
• Maintain knowledge and competency in nursing practice, engaging in mentoring/orientation of staff.
• Consider safety, cost, and impact factors in care delivery, providing leadership under RN direction.
• Demonstrate knowledge and competency in professional practice.

Requirements of the LPN/LVN:

• NY LPN Licensure
• BLS Certification (AHA or RQI required)
• 1 year experience as LPN, preferably in a Long Term Care facility
