The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. The LPN should possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.

The LPN is responsible for providing care needed by adult and geriatric patient groups. The role involves providing direct and indirect skilled nursing care for assigned residents. The LPN follows defined policies and procedures under the supervision of a Registered Nurse. A collaborative relationship must be maintained with other nursing staff, physicians, and ancillary staff. The LPN is expected to maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.

The ability to provide care to geriatric residents is essential. Key job duties include collecting data relevant to the resident's physical and psychosocial condition on an ongoing basis. Findings should be communicated to the Registered Nurse and other members of the interdisciplinary team. The LPN contributes to the creation of developmentally age-appropriate and culturally relevant plans of resident care. The plans should reflect nursing diagnoses and interventions to achieve desired resident outcomes.

Involvement of the resident/family and healthcare team in this process is crucial to promote a healthy and safe environment. Recommendations for revisions to the plan of care should be made after evaluation of resident outcomes. The LPN indicates the need for referrals to ensure all aspects of the resident's care have been addressed. Collaboration with the resident/family and members of the interdisciplinary team is necessary to assure customer satisfaction.

Maintaining current knowledge and competency in nursing practice is required. The LPN participates in the mentoring and orientation of staff. Consideration for factors related to safety, cost, and impact should be part of the planning and delivery of care. Under the direction of the Registered Nurse, the LPN provides leadership in the professional practice setting. Demonstration of knowledge and competency is expected.

Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
