The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the life span. The LPN must possess the ability to assess data reflective of the patient's status. They should interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs.

The LPN will provide care needed by adult and geriatric patient groups. They will provide direct and indirect skilled nursing care for assigned residents. The LPN follows defined policies and procedures under the supervision of a Registered Nurse. They must maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff. The LPN is expected to maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.

They must be able to provide care to geriatric residents.

Job Duties:

The LPN will collect data relevant to the resident's physical and psychosocial condition on an ongoing basis. They will communicate findings to the Registered Nurse and other members of the interdisciplinary team. The LPN contributes to the creation of a developmentally appropriate and culturally relevant plan of resident care. This plan should reflect nursing diagnosis and interventions to achieve desired resident outcomes involving the resident, family, and healthcare team.

They will promote a healthy and safe environment. The LPN will recommend revisions to the plan of care after evaluating resident outcomes. They will indicate the need for referrals to ensure all aspects of the resident's care have been addressed. Collaboration with the resident, family, and members of the interdisciplinary team is essential to ensure customer satisfaction. The LPN must maintain current knowledge and competency in nursing practice.

They will participate in the mentoring or orientation of staff. Factors related to safety, cost, and impact should be considered in planning and delivering care. Under the direction of the Registered Nurse, the LPN will provide leadership in the professional practice setting. They are expected to demonstrate knowledge and competency. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
