Position: LPN / LVN - LTC

LPN Part Time Evenings

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development and possess the ability to assess data reflective of the patient's status. Direct and indirect skilled nursing care will be provided for assigned residents. The LPN will follow defined policies and procedures under the supervision of a Registered Nurse.

A collaborative relationship will be maintained with other nursing staff, physicians, and ancillary staff. The LPN must maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. Responsibilities include collecting data relevant to residents' physical and psychosocial conditions on an ongoing basis. Findings will be communicated to the Registered Nurse and other Interdisciplinary Team members.

The LPN will contribute to the creation of a developmentally appropriate and culturally relevant care plan. This plan will reflect nursing diagnosis and interventions to achieve desired resident outcomes. The LPN will involve the resident/family and healthcare team in this process while promoting a healthy, safe environment. Revisions to the care plan will be recommended after evaluating resident outcomes. The LPN will ensure all aspects of resident care have been addressed and collaborate with the resident/family and Interdisciplinary Team.

Current knowledge and competency in nursing practice must be maintained. Participation in the mentoring/orientation of staff is expected. The LPN will consider factors related to safety, cost, and impact in planning care delivery. Under the direction of the Registered Nurse, the LPN will provide leadership in the professional practice setting. Knowledge and competency must be demonstrated. Occupational exposure to blood borne pathogens is reasonably anticipated.

Required tasks involve a potential contact with blood, body fluids, or tissues.
