Schedule:

Floating Shift 5x8s-Hour 40hours/week

• Schedule: Full-Time, Days/Nights

• Weekend: Every other weekend (EOW)

• Experience: 1 year LPN experience required

• License: New York LPN License required

• Certification: BLS — AHA or RQI only

• Patient Population: Adult and geriatric residents

• Reporting: Works under the supervision/direction of an RN

• Key Responsibilities:

  • Provide direct and indirect skilled nursing care to assigned residents

  • Assess and collect ongoing physical and psychosocial data

  • Communicate resident findings to RN and Interdisciplinary Team

  • Contribute to individualized, age-appropriate and culturally relevant care plans

  • Evaluate resident outcomes and recommend care-plan revisions

  • Identify need for referrals and collaborate with residents, families, and care team

  • Maintain nursing knowledge and clinical competency through continuing education

  • Participate in staff mentoring/orientation

  • Promote a safe and healthy care environment

  • Provide leadership within scope of practice under RN direction

• Additional: Must demonstrate competency in caring for geriatric residents and understanding age-specific needs

• Exposure: Potential occupational exposure to blood borne pathogens, body fluids, and tissues

Skills:

Skill Required / Desired Amount of Experience Expertise Rating Move NY LPN Licensure, BLS Required Experience serving in an LPN capacity at LTC Facility Required 1 Years
