Care Manager, Village Volunteers

Village Volunteers, a nonprofit organization dedicated to helping older adults age in place with dignity, independence, and purpose, is seeking a compassionate and organized Care Manager to join our team serving Coastal Sussex County.

The Care Manager supports older adults by conducting in-home assessments, monitoring member needs, coordinating services and referrals, advocating for member health and safety, and collaborating with families, volunteers, healthcare providers, and community partners. This position also provides leadership to the Care Team and helps ensure members remain connected, supported, and independent in their homes and communities.

Qualified candidates will have experience in healthcare, social work, case management, aging services, or a related field; excellent communication and organizational skills; and a passion for serving older adults. A degree in social work, nursing, human services, gerontology, healthcare administration, or a related field is preferred. Licensed Social Workers (LSW/LCSW) and Registered Nurses (RN) are encouraged to apply.
