Physical Therapist (PT) - Must Be Licensed

A great SNF / LTC is looking for an experienced or newly graduated, hard‑working Physical Therapist / PT to join their growing team for a contract assignment.

• Conduct assessment of referred patients requiring rehabilitation services, including direct patient care assessment of medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to patient capacity and tolerance under the direction of the physician.
• Set realistic and achievable goals for patients, document and record patient conditions, and educate patients and families on appropriate physical therapy methods.

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT, or be proactively in the process of applying for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
