Position Opening – Agency with Choice Coordinator

Easterseals Delaware & Maryland’s Eastern Shore is seeking an AWC Coordinator to be a part of our Agency with Choice Program (AWC) in the Sussex county area. The AWC Program assists individuals and their families under the Lifespan Waiver through the Division of Developmental Disabilities Services with the hiring and management of their caregiver(s). We are looking for someone who is well-organized, has effective time management skills, and excellent customer service.

Qualifications:

Exhibit highly effective organizational and administrative skills.
Excellent internal and external customer service
Experience in serving individuals with disabilities and engaging multiple stakeholders

Able to manage multiple task and projects.
Successful in developing high volume of detailed and comprehensive case notes

Job Duties:

Enrolling, monitoring, and assisting participants, families, and their caregivers.
Assist with balancing and managing the participants funding for the year.
Train the participant and caregiver on the electronic visit verification system,Therap.
Assist Participants and Caregivers with payroll and time reporting troubleshooting and solutions
Meet with participant and their family quarterly and as needed regarding program services and processes.
Collaborate with Division of Developmental Disabilities Services regarding participant services and support.

Preferred Qualifications :

Prior experience with disabled and/or aging population
Bachelor’s Degree or 3 years’ experience
Bilingual ability in Spanish is aplus.
Experience using the Therap system is a plus.

This is a full-time, 40-hour a week position Monday
- Friday 8am-4pm. There is county-wide travel associated with this position. Must have reliable transportation. The salary for this role will be based on experience in the field.
