Job Description
We are seeking qualified Licensed Practical Nurse LPN candidates interested in joining our Assertive Community Treatment (ACT) team at our Georgetown, Delaware location.
The Licensed Practical Nurse LPN as part of the ACT multidisciplinary team provides the coordination of both the behavioral and physical health care needs of the program participants, monitors participants medication and provides psychiatric nursing services consistent with physician’s orders. Some of the responsibilities include the following.
- Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.
- Maintains a case load of assigned participants and completes assigned paperwork in a timely manner.
- Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.
- Provides ongoing assessment of participant’s mental health symptoms and responds to recovery plans.
- Educates and supervises participants in self-administration of medication.
- Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.
Job Qualifications:
- Board Certified Licensed Practical Nurse (LPN) in the state of Delaware; or participating in the Nursing Licensure Compact.
- Knowledge of mental health systems and supports.
- Knowledge of multiple support systems for substance abusers.
- Good communication skills, both oral and written.
- Ability to work independently and as part of a focused team.
- Possession of a valid driver's license with an acceptable driving record.
- Very strong organizational and analytical skills.
Scope:
- Frequent talking and listening.
- Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants.
- Local travel required, including transporting participants.
- May be required to report to work during emergencies, including inclement weather.
- May require crisis intervention in emergency situations.
- Scheduling flexibility and possible on-call work are necessary.
