Registered Nurse (Consultative) Field - IDD Services
Hours: Part‑time (10 to 12 hours per week, Monday‑Friday, 8:30 a.m.–4:00 p.m. – varies)
Hourly Rate: $33 per hour
Location: Georgetown, DE (Kent & Sussex County)
The Registered Nurse (Mobile, Hourly) is responsible for providing nursing services, training, and support to address the medical or psychiatric needs of individuals with developmental disabilities, acquired brain injuries, substance abuse issues, and/or emotional or behavioral challenges across multiple sites. The RN will travel between locations, perform duties, and remain accessible to staff during working hours. Occasional after‑hour availability for consolation may be required.
EssentialJob Functions
- Conduct a medical assessment of each individual referred to the program and collaborate with intake staff to determine appropriate placement in the community.
- Participate in the development and implementation of service plans and supervise, in conjunction with physicians, the medical components of the plan.
- Review documents and revise all nursing‑related care plans regularly.
- Ensure medical records and reports are current and complete in accordance with regulatory and corporate standards, including medication administration records and physicians’ orders.
- Maintain contact with primary care physicians and specialists; when directed, attend doctor visits with the mentor and/or individual receiving services.
- Promote health and wellness for individuals and ensure nursing actions and steps are put in place for prevention.
- Administer medications according to established guidelines.
- Provide documentation for all medications given and review medication administration records for compliance.
- Oversee the management of high‑risk medical conditions and private duty nursing services.
- Develop training flow charts specific to each individual; provide and document medical skills training to clients, mentors, respite providers, staff, and, where appropriate, the biological family.
- Present medical skills components at Pre‑service Orientation.
- Serve as a member of the Individual Health Plan (IHP) or Plan of Care (POC) team.
- Evaluate the medical needs of each individual for initial IHP/POC.
- Ensure IHP/POC goals and objectives are medically indicated.
- Generate ongoing health care protocols for the specific medical conditions outlined in the IHP/POC.
- Provide consultation and technical assistance to staff and mentors regarding home care issues and the operation of durable medical equipment.
- May be required to coordinate, organize, and/or assist with household activities such as light housekeeping and meal preparation; transport individuals to planned and/or necessary activities and appointments.
- Participate in an on‑call system as needed.
- Travel between multiple locations during work hours; provide support and remain accessible to staff at all locations during working hours.
- May be required to be accessible after hours for consolation.
- May actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee, and Administration meetings.

Education and Experience
- Graduate of an accredited RN program.
- One year experience working with medically complex individuals.
- Current state RN License.
- Current driver’s license, car registration, and auto insurance if driving on the behalf of the company or providing transportation to individuals receiving services.
- Current CPR/First Aid certification.
- Job requires duties that involve potential exposure to blood, body fluids, or tissues. Tasks that involve exposure are an expectation of employment.
- Heavy work. Exerting up to 100 pounds of force occasionally, up to 50 pounds frequently, and up to 20 pounds constantly to move objects.
- Travels between multiple locations.
