We are actively seeking a talented Hospice Registered Nurse (RN) Case Manager for 40 hours per week
.
The Hospice Registered Nurse (RN) Case Manager functions as a member of the interdisciplinary team to provide coordination of interdisciplinary hospice services to patients and families. Utilizes the nursing process to provide quality nursing care to Hospice patients in accordance with company policy. Supports the primary caregiver in the performance of physical care for the patient. This is an exempt position.
Qualifications- NEW GRADS are welcome to apply! Hospice/homecare experience not required. Come to us with the Hospice Heart and we will train you!
- 2 years of oncology, medical-surgical or home care nursing experience preferred
- Current Registered Nurse license to practice professional nursing in the State of Delaware
- Valid Driver’s license
- Graduate of an approved school of nursing (BSN preferred but not required)
- CPR certification required

- Monday – Friday; 8:00 am‑4:30 pm; 1 weekend day per month (NO On‑Call)
- Providing hospice care to patients in Sussex County in both homes and facilities (will be assigned a centralized territory to reduce windshield time)
- 4 visits per day on average (will carry an average case load of 14‑16 patients)
- $83,000 - $89,000 per year
- Mileage reimbursed at $0.67 per mile
