Per Diem Nurse Practitioner Home Health | Georgetown, Delaware

Work with Medicare Advantage plan members residing throughout Sussex County, DE including areas such as Milford, Seaford, and Georgetown. • Flexible shifts offered seven days a week (Monday-Sunday)
• Create your own schedule to fit your availability
• Choose a minimum weekly or bi-weekly hourly commitment

• Visit patients in their homes to complete annual health risk assessments
• Evaluate patients prior and current medical history, conditions, and medication use
• Provide education to patients regarding their health conditions and medications
• Identify potential gaps in care and recommend appropriate next steps
• Document findings without responsibility for treatment planning or medication prescribing

• Valid Nurse Practitioner license in Delaware
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• Current CPR certification (BLS)
• At least six months of recent Nurse Practitioner experience preferred

This per diem position offers compensation of $80-$90 per completed evaluation, plus a mileage stipend and pay for administrative time. Each in-home visit typically takes about 45 minutes, and your assignments may vary between annual health assessments and post-discharge follow-ups.
