Work with Medicare Advantage plan members residing throughout Sussex County, DE including areas such as Milford, Seaford, and Georgetown. As a valued Nurse Practitioner in this role, you will make a direct impact by helping to improve health outcomes for individuals in your community, all while enjoying a flexible work schedule.
Schedule- Flexible shifts offered seven days a week (Monday-Sunday)
- Create your own schedule to fit your availability
- Choose a minimum weekly or bi-weekly hourly commitment
- Visit patients in their homes to complete annual health risk assessments
- Evaluate patients prior and current medical history, conditions, and medication use
- Provide education to patients regarding their health conditions and medications
- Identify potential gaps in care and recommend appropriate next steps
- Document findings without responsibility for treatment planning or medication prescribing
- Valid Nurse Practitioner license in Delaware
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR certification (BLS)
- At least six months of recent Nurse Practitioner experience preferred
This per diem position offers compensation of $80-$90 per completed evaluation, plus a mileage stipend and pay for administrative time. Each in-home visit typically takes about 45 minutes, and your assignments may vary between annual health assessments and post-discharge follow-ups.
