Position: Registered Nurse – Hospice

$2102.56/week -

Job Summary:

Provides comprehensive hospice home nursing care to patients according to physician orders, ensuring quality and compassionate services. Coordinates patient care activities, supervises home health staff, and maintains adherence to agency policies and procedures.

Responsibilities:

  • Deliver skilled nursing care to hospice patients in their homes, following physicians’ prescriptions and treatment plans.
  • Coordinate and oversee patient visits, documentation, and communication with the interdisciplinary team.
  • Supervise and evaluate Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses.
  • Ensure all clinical documentation is completed accurately within required time frames, utilizing Home Care Home Base.
  • Maintain compliance with travel and licensing requirements, including proficiency in OASIS and EMR systems.
  • Participate in office meetings and training sessions as needed to stay current on hospice care standards.

Qualifications:

  • Require a valid RN license in good standing with current licensure.
  • Possess at least 1 year of recent hospice or adult home health experience.
  • Demonstrate proficiency in Home Care Home Base EMR system and, for RNs, in OASIS documentation.
  • Have reliable transportation, a current driver’s license, and agency-required liability insurance.
  • Be willing to travel up to 75 miles from the care center.
  • Able to commit to working schedule requirements, including potential on-call rotations.
