Job Summary:

The Nurse Navigator works closely with Physicians to coordinate a patient’s care. Under medical and administrative direction, they provide patient education related to their illness and planned treatments. The Nurse Navigator provides leadership and nursing services to patients in our community health setting and works with the health care team of medical providers to provide quality health care services for our patients.

They work closely with the CMO, Providers, and Chief Operations Officer.

Job Responsibilities:

Listed are a representative summary of the major duties and responsibilities of a nurse navigator at La Red Health Center. This is not a comprehensive or exhaustive list or all job functions.

  • Works with a multi-disciplinary team to develop and implement an up-to-date care plan.
  • They use clinical protocols to refer patients to the appropriate specialist for care and treatment.
  • They help patients and their families in obtaining referrals to a specific specialist.
  • ’They provide emotional support and counseling, related to specific clinical situation.
  • They work with designated physicians to develop and maintain care protocols.
  • They communicate with referring physician’s office as required.
  • Provides patient care based on practice guidelines standards of care and federal/state laws and regulations.
  • Assures that all EMR documentation is complete as required.
  • Participates in multidisciplinary teams to improve patient care processes and outcomes and quality improvement.

Requirements:

Education and/or

Experience:

  • LPN with 3+ years of nursing experience.
  • Candidate must be a licensed in the state of Delaware.
  • Three plus years nursing experience in a private practice, hospital or health center setting.

Language

Skills:

Bilingual skills preferred (English/Spanish)

Performance Requirements:

Knowledge:

  • Knowledge of nursing processes, health care systems, structure and functions; community resources, hospitals. Understanding of medical technology, equipment and supplies.
  • Knowledge of wellness, illness, human behavior, psychosocial factors and alternative health care treatments that are ethnically predominant.
  • Knowledge of medical record documentation requirements, and federal/state laws regarding release of medical records or health care information

Skills:

  • Skill in time management, problem solving, multitasking, prioritizing and medical care coordination.
  • Skill in initiating appropriate crisis interventions and emergency response
  • Skills in patient triage in person and on the phone.
  • Skill in dealing with large quantities of information in an organized manner.
  • Skill in using computer and medical records software.
  • Skill in posting all pertinent information, laboratory and test results information into EMR, meeting Center standards for timeliness.

Abilities:

  • Ability to analyze options and counsel patients and families about choices and make referrals to other providers and resources.
  • Ability to engage nurses and clinical staff in implementation of PCMH standards.
  • Ability to read and interpret physician orders and notes.
  • Ability to use excellent interpersonal skills in building rapport and teamwork.
  • Ability to review and analyze data.
  • Ability to analyze medical records for completeness and accuracy, paying attention to detail.
  • Ability to manage time and appointment scheduling effectively

Equipment Operated:

Wide range of standard medical equipment including oxygen tanks, wheelchairs, thermometers, etc. Computer use and proficiency required.

Work Environment:

Medical exam/treatment rooms and medical offices in Health Center. Controlled lighting, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.

Mental/

Physical Requirements:

Involves standing, walking, kneeling, bending, grasping, manipulating and squatting. Walking or standing may be more than four (4) hours per day.
