Signify Health, part of CVS Health, is seeking a Per diem Clinician (NP or PA) to provide in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the health plan. Flexible, part-time hours with travel within licensed states are offered.
