Work Schedule:

• Full Time (38 hours weekly)
• M-F with a Saturday morning rotation

Job Requirements:

• Graduate of an approved technical, professional, or vocational program in Healthcare;
  Healthcare clinical experience preferred physician practice or related field
• Equivalent experience accepted in lieu of degree
• Must be certified or registered medical assistant
• Must maintain certification and registration status:
  Registered Medical Assistant (RMA) through AMT, NCCT, or NHA, or certified Medical Assistant (CAN) by AAMA, and Cardiopulmonary Resuscitation (CPR)
• Medical office flow, especially the clerical/front office tasks
• Ability to make quick decisions based on well thought out consequences/results
• Knowledge of EMR, practice management software, and medical coding/billing strongly encouraged

Job Overview:

This position provides direct patient care in a primary care office and collaborates with care delivery providers to identify gaps in care, schedule required visits, and provide referral follow‑up. The Medical Home Medical Assistant assists with pre‑visit planning, coordinates electronic portal messages, and supports transitions of care. The role acts as a clinical liaison to the physician care plan, actively communicates with patients, and participates in process improvements focused on patient satisfaction and engagement.

Job Responsibilities:

• Demonstrate proficiency in the rooming process for adult and/or pediatric primary care, including clinical procedures, immunizations, venipuncture, and point‑of‑care testing.
• Follow protocols and policies for scheduling, clinical procedures, and appropriate use of medical equipment.
• Provide accurate documentation of clinical calls, patient rooming information, and order entry; manage prescriptions, pharmacy information, and workflow edits.
• Address patient messages (phone, MyChart) promptly and appropriately elevate issues.
• Maintain knowledge of population health and engage in strategies to close gaps in care, including pre‑visit planning and daily huddles.
• Apply knowledge of the adult and/or pediatric preventative medicine schedule, immunizations, screening tools, and patient education for all served patients.
• Collaborate with wrap‑around services—behavioral health, complex care nursing, social workers, community health workers—to provide community resources and support patient coaching.
