Are you a compassionate Physical Therapist looking for flexibility, autonomy, and the opportunity to make a meaningful impact? Join a growing home healthcare team where you'll build one-on-one relationships with patients while helping them regain independence in the comfort of their own homes.

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services in patients' homes
• Monitor progress and modify care plans as needed
• Educate patients and caregivers on exercises, safety, and daily living activities
• Collaborate with physicians, nurses, and other healthcare professionals to ensure the best patient outcomes
• Complete required clinical documentation, including OASIS assessments and progress notes
• Supervise Physical Therapy Assistants as appropriate

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license in the State of Ohio
• Home health experience is preferred but not required
• Two years of PT experience preferred
• Strong communication, organization, and clinical decision-making skills
• Ability to work independently while providing exceptional patient care
