Registered Physical Therapist (PT) – Home Health
Location:
Eastern Cincinnati Area (including parts of Brown & Adams Counties)
Earn $60-$110 per visit

Are you a compassionate Physical Therapist looking for flexibility, autonomy, and the opportunity to make a meaningful impact? Join a growing home healthcare team where you'll build one-on-one relationships with patients while helping them regain independence in the comfort of their own homes.
What You'll Do- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy services in patients' homes
- Monitor progress and modify care plans as needed
- Educate patients and caregivers on exercises, safety, and daily living activities
- Collaborate with physicians, nurses, and other healthcare professionals to ensure the best patient outcomes
- Complete required clinical documentation, including OASIS assessments and progress notes
- Supervise Physical Therapy Assistants as appropriate
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the State of Ohio
- Home health experience is preferred but not required
- Two years of PT experience preferred
- Strong communication, organization, and clinical decision-making skills
- Ability to work independently while providing exceptional patient care
