Position: RN Case Manager - Home Health Agency (PT or FT: Weekdays, Daytime)

Home Health RN Case Manager & Recertification Nurse (RNCM)

Job Summary – As an RN Recertification Nurse, you will coordinate, re‑evaluate, and confirm the medical necessity of continued home health care services for our patients, ensuring optimal outcomes. You will manage assessments, documentation, and communication with physicians, agency staff, and clients.

Service Area
  • Primary / Centralized:
    Dayton‑Middletown‑Springfield Metro
    , Washington Court House‑Wilmington Metro
  • Secondary (less frequent travel): all Southwest Ohio and Mid‑Central Ohio communities, including Dayton, Middletown, Springfield, Piqua, Urbana, Cincinnati & Cincy Metro areas, Washington Court House, Salem Township, Wilmington, Circleville, Columbus & immediate Columbus Metro area.
  • Qualified applicants may reside anywhere within the service area provided travel requirements are met. Overnight travel is not required.
Employment & Schedule Type
  • Long‑term employment, Part‑Time or Full‑Time RNCM/Recert Nurses.
  • Both capacities require weekday availability (Monday‑Friday) 8 am‑5 pm, occasional/rare evening availability scheduled in advance.
  • Full‑time:
    Salary, travel throughout entire region (up to ~2‑hour daily radius).
  • Part‑time:
    Hourly, travel throughout sub‑region (minimum 1‑hour radius; travel up to ~2‑hour daily radius highly desirable).
  • No on‑call shifts; weekend services optional when available but not required.

Compensation
  • Full‑time annual salary (plus PTO and mileage) starts at $78,000–$83,200 per year, based on travel radius and minimum service volume, with quarterly bonus opportunities.
  • Part‑time hourly wage (plus mileage) starts at $35–$40 per hour, based on minimum 1‑hour travel radius.
Work Responsibilities
  • Perform client assessments; plan and initiate appropriate actions in home care settings.
  • Manage and supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document, communicate, and coordinate patient status changes to the care team.
  • Provide skilled nursing visits and services when needed, including medication sets and dressing changes.
  • Remain available to travel to surrounding counties as required.
  • Ensure compliance with state, federal, and agency guidelines for documentation and care delivery.
Reports To

Regional Program Administrator, or Clinical Administrator/Director of Nursing.

Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum 1 year experience as a Registered Nurse with an active RN license in Ohio.
  • Minimum 1 year experience in a home health agency or equivalent setting.
  • CPR and First Aid certifications (obtainable before hire).
  • Experience with OASIS and NON‑OASIS assessments.
  • Strong background in patient care, confidentiality, and documentation.
  • Excellent attendance and punctuality; consistent availability aligned to chosen schedule type (PT/FT/Hybrid).
  • Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, and positive.
  • Knowledge of HIPAA regulations and medical terminology.
  • Ability to meet required physical demands (see below).
  • Consent to and ability to pass a state and federal background check (including fingerprints).
  • 18+, active driver’s license/state , and US work authorization.
  • Reliable transportation within full travel radius of assigned clients (minimum 2‑hour reach).
